After we have received your completed application, along with all required information and documentation, we will contact you about your admissions decision. Complete Form FSA-4.

Purchase a Health Insurance Plan offered through HTH/SUNY (required for participation in the International Program).

Submit proof of immunization and proof of vaccination.

Provide photo identification.
